During my tenure as a Clinical Nurse Specialist (CNS), I encountered a critical situation involving a patient experiencing a respiratory distress emergency. It was a busy afternoon in the emergency department when an elderly patient was rushed in by paramedics with severe difficulty breathing. The patient had a history of chronic obstructive pulmonary disease (COPD) and was visibly distressed and struggling for each breath.
Without wasting any time, I swiftly assessed the patient’s vital signs, which revealed a rapid heart rate, increased respiratory rate, and decreased oxygen saturation levels. Recognizing the seriousness of the situation, I immediately initiated the appropriate interventions. Firstly, I ensured the patient had a patent airway and administered supplemental oxygen to improve oxygenation.
Given the patient’s history of COPD, I suspected an acute exacerbation. I collaborated with the medical team to order a chest x-ray, arterial blood gas analysis, and additional blood work to identify the underlying cause of the exacerbation. While waiting for the results, I closely monitored the patient’s respiratory status, continuously assessing their oxygen saturation levels, lung sounds, and overall level of distress.
