Presenting Complaint: Mrs. Smith presented with shortness of breath, fatigue, and edema in her lower extremities.

Assessment: Physical examination revealed jugular venous distension, an S3 gallop, and pitting edema in her legs. Chest x-ray showed cardiomegaly and pulmonary edema.

Diagnosis: Acute decompensated heart failure

Challenges:

  • Comorbidities: Mrs. Smith’s multiple comorbidities added complexity to her condition.
  • Medication adherence: She had difficulty remembering to take her medications consistently.
  • Limited support system: Mrs. Smith lived alone and had minimal family support.
